An increased risk of injuries in the elderly are related to muscular skeletal weakness, inappropriate footwear, diminished vision, mental status changes, medications, and environmental dangers. This is demonstrated by a fall rate of 30%.

  1. Explain how the health concern from your community health nursing diagnostic statement is linked to a health inequity or health disparity within the target population. There is a health disparity in Baltimore City between elders that have a support system and those who do not. The population of non-supported elders are growing. Baltimore City has a population roughly of 250,000 residents. Out of this number, there are approximately 85, senior citizens. The disparity of the group is manifested by a lack of a support system and being poor economically.

Many of the residents that are poor and uninsured do not stay in the hospital long after a fall. Usually, the elders that are insured stay longer and their health outcome is better. The disparity against poor uninsured often takes place because medical institutions know they cannot pay the high hospital cost. Therefore, their hospital stay is shortened. As a result of the poor and uninsured being discharged prematurely, they do not have the physical strength or therapeutic training to sustain from future falls.

Additionally, black men wait longer for an ambulance to arrive at the scene of a fall. The media has shone a negative light on the black male. Therefore, the disparity against this race and gender is great. When there is a fall and or injury emergency medical technicians’ respond slowly to the scene. This is due to the stereotype that black men are violent and do not follow medical advice.

  1. Discuss the evidence-based practice associated with the Field Experience topic.

The state of Maryland undertook a systematic examination of formative and summative evaluations and assessments based on older people’s perspectives on fall prevention. The review was conducted to supplement the clinical efficacy evidence supplied by an evaluation conducted by Maryland’s Aging Locals Organization (MALO) on fall prevention. To raise awareness of a national guideline for the prevention of falls among the elderly. There were 9,518 senior persons aged 65 and up among them. In the state of Maryland, falls were the primary cause of injury-related hospitalizations and urgent care visits for this age bracket. In Maryland, 525 individuals aged 65 and up died as a result of falls in 2018.

This can be a tool for them to connect with one another. They can connect to discuss previous falls and how their life has changed as a result of them. They can also support each other in the challenges of their physical changes. Another page can also be utilized to connect with Physical and Occupational Therapist. Question can be asked about senior’s health and wellness. Tactics and advice for staying physically healthy can also be provided. YouTube can be used to display strength-training workout videos as well as sequences on safe and risky circumstances. a. Discuss the benefits of the selected social media platform in supporting preventative healthcare. There are several benefits in using the social media platform of YouTube to support fall prevention for the elderly population. Firstly, it is convenient. The senior can view this platform in the comfort of their home. Secondly, strength-training workout videos and other pertinent

information can be viewed, paused and rewind. Thirdly, the elder has the privilege of viewing when they like and as many times as they like. Fourthly, he or she can subscribe to favorite channels. There is always

up-to-date information being uploaded. So, the elder has 24-hour access to new videos as they are posted. 4. Discuss how the target population will benefit from your health message. The target audience will benefit greatly from my health message. Facebook is really easy to use, and anyone can sign up for free. A major benefit of the Facebook platform is that the individuals’ page can be made private. The app can be downloaded to a smartphone, tablet or watched straight through their website on laptops and PCs. You can also watch videos for free on YouTube. It also includes pre-roll commercials aimed towards the aging population and the dangers of falling. Information about fall safety practices as well as the crisis hotline. Moreover, any posted or seen YouTube video can be shared on Facebook, connecting the two channels and increasing the quantity of who see the actions. Over all, a healthier way of living would be a benefit to the focused group. A disciplined, structured life style will insert respectable routines that would become their new normal. D. Describe best practices for implementing social media tools for health marketing. Providing health marketing on social could be a fragile risk. Several factors must be taken into consideration as this platform can convey an unintended message. Additionally, the message may

resolving and coping with issues that arise in their daily lives. As a stakeholder the social worker will formulate a Facebook resource group. There will be 10 commercials/advertisements that would appear every hour. Theses post would intel valuable information of community exercise groups, housing with no stairs, healthy eating and wellness care. All of the stakeholders will have different roles and responsibilities, but one common goal, fall prevention.

  1. Discuss potential public and private partnerships that could be formed to aid in the implementation of your campaign. I would seek public and private partnerships to aid in my campaign. I’d develop prospective public partnerships with local community associations. Some examples would be the city councilmen, state funded adult day care centers, and the health department. Forming a partnership with city councilmen would be a great advertisement avenue. As the city councilmen address the needs of the community, they could use Facebook to target the geriatric population. This can be done with fall prevention ads supported with their image. Adult day care centers partnerships could support my campaign by tweeting about it via the adult day care twitter account. The health department could aid in the implementation of my campaign by advertising via YouTube. The health department could make fall video scenarios. The health department is a trusted community organization that many people would pay close attention to. I’ll develop private relationships with churches and local mason chapters. The churches could put advertisements on their Facebook page in the senior’s bulletin. Lastly, local mason chapters could advertise my campaign on twitter. Many of the masons are older

gentlemen. So, by partnering with the organization the campaign provides information to its members that are 65 and older. 3. Create a specific timeline for implementing your campaign. Before the time line is started three phases would be taken into consideration. Planning, implementation and evaluation phase. Both private and public partners will advance through the designated time period with process of these phases. The timeline would roughly be four weeks. I would start with the churches. Generally church members spread the word about things in the community quickly. Church members are often trusted people.

10 Then I would focus my attention toward the health department. These two potential partnerships will take approximately five days. Once my message has been received and accepted, I will launch the information on Facebook. This would take about a day. Next, I would connect with my local adult daycare centers. This would take approximately two days. Involving the city council will take about five days. Contacting the city council will take about five days. Lastly, it will take approximately six days to connect with the local mason chapters. At the end of the forementioned days there will be an evaluation to see if the campaign is headed in a successful direction. 4. Explain how you will evaluate the effectiveness of the campaign. To evaluate the effectiveness of the campaign I would contact the health department and hospitals. I would ask both health department and hospitals administrators to present six months’ worth of data that disclose hospital visits and admissions that involved falls. By obtaining the previous six-

    More than one out of every five senior citizens fall each year, but just half of those who do so tell
    their doctor. The likelihood of an elder falling doubles after an initial fall. To bring awareness to
    the community, many local programs are in place. The programs are designed to help
    Marylanders understand the risks and long-term disadvantages of falls. Maryland’s Violence and
    Injury Prevention Program assist the local health department with resources.
    These resources are used to educate families and businesses on how to make the environment
    safe for the elderly population. Maryland Department of Health (MDH) provides monthly
    internet-based seminars with the target of fall prevention. During Fall Prevention Awareness
    Week, Maryland’s Free Fall Coalition also hosts activities in the community such as instructional
    lectures, locomotor and coordination seminars, home risk monitoring, ambulatory device
    demonstrations, and health checks.

    The contributing factors that often causes falls in the elderly population are musculoskeletal
    disorders, vision complications, environmental dangers, improper footwear, and medication
    usage. Often the elderly person suffers from unsteady balance or lower extremity weakness.
    These factors cause the person to frequently fall. Vision problems is another concern that cause
    the aged person to fall. Poor vision is another reason that contributes to falls. Vision problems are
    not just caused by the aging process, but disease development as well.
    An example of this is glaucoma. Environmental threats are clutter or scattered rugs. Falls that
    results from environmental clutter often results in bone fractures. The wearing of improperly

    The target audience will benefit greatly from my health message. Facebook is really easy to use,
    and anyone can sign up for free. A major benefit of the Facebook platform is that the individuals’
    page can be made private. The app can be downloaded to a smartphone, tablet or watched
    straight through their website on laptops and PCs. You can also watch videos for free on
    YouTube.
    It also includes pre-roll commercials aimed towards the aging population and the dangers of
    falling. Information about fall safety practices as well as the crisis hotline. Moreover, any posted
    or seen YouTube video can be shared on Facebook, connecting the two channels and increasing
    the quantity of who see the actions. Over all, a healthier way of living would be a benefit to the
    focused group. A disciplined, structured life style will insert respectable routines that would
    become their new normal.
    D. Describe best practices for implementing social media tools for health marketing.
    Providing health marketing on social could be a fragile risk. Several factors must be taken into
    consideration as this platform can convey an unintended message. Additionally, the message may
    be lost in the verbal or visional busyness of the graphics. One best practice that I think is
    important is honesty. Giving creditable information is vital in maintaining a faithful audience
    with a trusting relationship. The information must also be engaging.
    The targeted population should find the data interesting and applicable. Also, the statistics should
    involve testimonies. Real life stories are often more trusted than catchy phrases. If the audience
    can self-identity, they are more likely to share the information with friends. Lastly, make the

    platform user friendly. Sharing a hyperlink or directing people to a different page is insufficient.
    Patients should be able to easily find a doctor, schedule an appointment, or learn more about a
    treatment.
    E. Create a social media campaign implementation plan by doing the following:

    I would enlist the assistance of stakeholders in building my social media campaign. For starters,
    I’d hire a technological specialist to help me design a phone app with video links. The tech
    specialist would also be on call 24 hours a day to handle any problems that may arise with the
    app or videos. Secondly, I’ll request the help of a physical therapist. The physical therapist
    responsibility is to strengthen muscles for walking. The physical therapist would model videos
    on YouTube.
    The audiovisuals would have the capability to be paused, rewind and forwarded. They would be
    categorized to target proper balance, fall prevention, and muscle restoration. Thirdly, the social
    worker will attend to the patient needs as a stakeholder by connecting the patient with

    resolving and coping with issues that arise in their daily lives. As a stakeholder the social worker
    will formulate a Facebook resource group. There will be 10 commercials/advertisements that

    groups, housing with no stairs, healthy eating and wellness care. All of the stakeholders will have
    different roles and responsibilities, but one common goal, fall prevention.

    I would seek public and private partnerships to aid in my campaign. I’d develop prospective
    public partnerships with local community associations. Some examples would be the city
    councilmen, state funded adult day care centers, and the health department. Forming a
    partnership with city councilmen would be a great advertisement avenue. As the city councilmen
    address the needs of the community, they could use Facebook to target the geriatric population.
    This can be done with fall prevention ads supported with their image.
    Adult day care centers partnerships could support my campaign by tweeting about it via the adult
    day care twitter account. The health department could aid in the implementation of my campaign
    by advertising via YouTube. The health department could make fall video scenarios. The health
    department is a trusted community organization that many people would pay close attention to.
    I’ll develop private relationships with churches and local mason chapters.
    The churches could put advertisements on their Facebook page in the senior’s bulletin. Lastly,
    local mason chapters could advertise my campaign on twitter. Many of the masons are older
    gentlemen. So, by partnering with the organization the campaign provides information to its
    members that are 65 and older.
  9. Create a specific timeline for implementing your campaign.
    Before the time line is started three phases would be taken into consideration. Planning,
    implementation and evaluation phase. Both private and public partners will advance through the
    designated time period with process of these phases. The timeline would roughly be four weeks.
    I would start with the churches. Generally church members spread the word about things in the
    community quickly. Church members are often trusted people.

    Then I would focus my attention toward the health department. These two potential partnerships
    will take approximately five days. Once my message has been received and accepted, I will
    launch the information on Facebook. This would take about a day. Next, I would connect with
    my local adult daycare centers. This would take approximately two days. Involving the city
    council will take about five days. Contacting the city council will take about five days. Lastly, it
    will take approximately six days to connect with the local mason chapters. At the end of the
    forementioned days there will be an evaluation to see if the campaign is headed in a successful
    direction.
  10. Explain how you will evaluate the effectiveness of the campaign.
    To evaluate the effectiveness of the campaign I would contact the health department and
    hospitals. I would ask both health department and hospitals administrators to present six months’
    worth of data that disclose hospital visits and admissions that involved falls. By obtaining the
    previous six-
    month data would allow me to determine if there was a 10% decrease within the previous six
    months. An additional avenue to obtain the records is online at Maryland Department of Health
    Medical Records. Both data sources would be compared for accuracy.
  11. Discuss the costs of implementing your campaign.
    The cost of the campaign is roughly $600.00. The partnerships for the campaign require no pay.
    However, marketing materials are needed. Ink and copier paper is needed for flyer distribution.
    Luncheons with partners will be a cost. Building a relationship with the partners are pertinent to
    the campaign’s success. Fueling my car to meet partners will be a part of the cost. Social media
    advertisement is free. Lasty, purchasing pens for the partners as a kind gesture. This is a way to
    say thank you.

    F. Reflect on how social media marketing supports the community health nurse’s efforts
    to promote healthier populations.
    The advantage of using digital marketing to promote a healthy community is that it can reach
    people locally, regionally, and even globally. Public health nurses work in neighborhoods,
    focusing on various areas in order to enhance the overall wellbeing of the population who live
    there. The public health nurse could use social media marketing to support their efforts. A
    community Facebook page could be created. The word of community health events could be
    posted periodically as well members could leave comments, ask questions and/or give
    suggestions.
    YouTube could be used by advertising eye catching videos of upcoming community events.
    Twitter could be used frequently by spotlighting actions, events and fairs that will be taking place
    in the community. These tweets could be updated several times a day with current information.
