Hello and welcome to this short audio training tutorial on the importance of nursing-sensitive quality indicators. Health care quality has been at the center of nursing since the time of
Florence nightingale, who pioneered the use of empirical evidence to gauge the impact of
nursing interventions on patient outcomes. Over the intervening centuries, measurement of
health care quality has undergone significant transformation, particularly in the 1970s due to
work done by the American Nurses Association. Other important developments in this area
include the widespread adoption of the quality assurance model and the of Donabedian’s
structure, process, and outcomes model.
The National Database of Nursing Quality IndicatorsTM (NDNQI®) is one of its kind
database that provides periodic updates about three critical Nurse Sensitive Quality Indicators:
structure, process, and outcome measurements used to evaluate nursing care at the unit level
(Waugh & Bergquist-Beringer, 2020). The aim of this database is to help nurses to improve care
quality and patient safety by providing comparative data on nursing interventions and the
relationship they have to patient outcomes. The data used in this process is national and based on
empirical evidence (Montalvo, 2017. Registered institutions work with NDNQI liaisons if they
have issues related to nursing measures or the database itself.
We have used the term Nurse Sensitive Quality Indicators earlier. This term refers to
metrics used to identify health care and structure processes, both of which directly affect care
outcomes. Nurse Sensitive Quality Indicators demonstrate the value of nursing’s contributions by
evaluating aspects of patient care and outcomes that are directly influenced by nursing practice.
The indicators articulate three elements of nursing care: process, structure, and outcomes/results.
Process indicators evaluate nursing interventions and methods of assessing patients. Job
satisfaction among nurses is also taken as a process indicator. Structure indicators measure things

like nurse staffing levels, the qualifications of nursing staff, and the knowledge and skills of
nursing staff. Outcome or results indicators measure patient outcomes which are influenced by
the quality and quantity of nursing care provided (The Sentinel Watch, 2011).
This audio recording focuses on the outcome indicators. This is because the expectations
of nursing has shifted from merely providing care to health promotion. As such, today’s nurses
have an economic and social obligation to measure care outcomes. According to the American
Nurses Association (ANA) Social Policy Statement 2010, the main purpose of nursing is for the
public good. Being granted the privilege to practice the art and science of nursing by the public
leads to an expectation of quality care by the public. Therefore, the social contract between
nurses and the public requires that practitioners self-regulate themselves to ensure quality
performance, and this requires evaluation of performance (Oostendorp et al., 2020). The
definition of nursing also includes the very outcomes that nurses are required to evaluate and
manage, since they are duty-bound to protect, promote, and optimize health and abilities, prevent
illnesses and injury, alleviate suffering through diagnosis and treatment of human response, and
advocate for the care of individuals, families, communities, and populations. Therefore, each
nurse is obliged and accountable to measure interventions and outcomes in the areas of
preventing illnesses and injury, alleviating suffering, and promoting health. (Jones, 2016).
Measuring outcome indicators is at the core of preventing unnecessary illnesses and
injuries such as pressure ulcers, patient falls, and hospital acquired infections. In this respect,
new nurses should familiarize themselves with the outcome indicator of health care quality
because it can help them in quality control (Silveira et al., 2015).
Let us now turn to the collection and dissemination of outcome quality indicator data in my
health care facility.

I got the opportunity to talk to my facility’s quality control officer, and I was able to get
insights into how my organization collects data on nursing outcome indicators. My organization
is fairly advanced when it comes to the adoption of health technologies, and has implemented a
sophisticated health information system that facilitates and enhances critical operations. In this
respect, the person I interviewed told me that the organization primarily relies in patient
electronic health records to measure the outcomes of care provided. They use medical records as
the main method of auditing quality as and a basis for designing improvement initiatives. Using
these records, the quality assurance team is able to review patients’ medical histories and
document their care and progress. At the hospital level, outcome indicators are used to support
quality improvement initiatives, as well as in monitoring and assessing adverse events related to
patient safety. Adverse events related to patient outcomes that are monitored and assessed using
hospital records include monthly mortality due to acute myocardial infarction, pneumonia, and
heart failure; Hospital Acquired Infections such as CLABSI and surgical site infections, and
CAUTIs; and complications such as pressure ulcers, Post-operative sepsis, Accidental puncture
or laceration, Post-operative wound dehiscence, falls, and Post-operative pulmonary embolism.
Aggregate data about nursing outcome indicators is released by the organization mainly through
electronic media in the form of PDF files and non-proprietary open systems.
Nurses have an essential role to play in assuring patient safety and better outcomes as
they undertake their primary duty of providing care. Unlike physicians who have limited contact
with patients, nurses maintain constant presence at the bedside and are better placed to monitor
and assess progress of treatment interventions. Nurses also interact with different classes of staff
such as physicians, relatives of patients, pharmacists, and other members of the health care team,
making them crucial to timely communication and coordination of patient’s condition to the

interdisciplinary healthcare team. Patients expect nurses to be at the frontline in monitoring
worsening clinical conditions, identifying near misses and errors, knowledge of care processes
and any weaknesses in these systems, detecting and communicating changes in patient
conditions, and carrying out innumerable other duties that ensure that the quality of care they get
is high. Nurses play a critical role in promoting high-quality care because they are involved in
nearly every aspect of health care quality, including caring for patients, managing medications
and bedside conditions, assisting physicians during surgeries, and gathering and reporting care
data (Kazana & Dolansky, 2021).
To conclude this audio training on the importance of nursing-sensitive quality indicators,
we have seen that current nursing principles encourage care providers to monitor the impact of
their interventions on three domains, structure, processes, and outcome indicators. This tutorial
has focused on outcome indicators, which are measurements that assess the impact of nursing
interventions on patient safety and outcomes, including better patient experience of care and the
prevention of adverse events like injuries and infections.
