Case presentation

Pt reports he has anxiety since he was discharged from the hospital. Even
though he told me in his last session he was feeling better after discharge from
hospital. Reported he had court cases so he told them he is feeling better so
they can discharge him early. We discussed titrating up medicine but pt is
focused on getting Klonopin. Reports that\\’s the only medicine helped him in PR.
He was counselled about side effects of Benzo, BB warning, alternatives. He
was resistant to increase the dose of current meds and was demanding
Klonopin and was threatening to see another doc who ll prescribe him Benzo.
Later he expressed his agreement on titrating up his current meds. We also
discussed crisis plan. He understood and agreed. Denies SI/HI/AVH and access
to weapons.
Per Intake: Pt recently discharged from Bridgeton behavioral health and came
to guidance ctr to continue out pt care. He was admitted for depression and SI.
He reported struggling with depression, anxiety and AH since age 26yrs and
has been admitted in PR 6 times. He recently moved to US in Feb 2022.
Reports feeling depressed and hearing voices to kill himself, so he admitted
himself in the hospital. He was off meds since he came to US.
Reports currently feeling fine, compliance with his discharge medications. Does
report having anxiety in the daytime and is requesting medication for that. He
was counselled about Visatril, risk/alternatives/SE are discussed. He expressed
understanding and agreement.
He lives with his brother in US. He is currently denying SI/SJI/AVH and
paranoia. Have reported manic and hypomanic symptoms in the past in PR.
Denies currently.
Patient was educated for their diagnoses and treatment plan. Pt was offered
supportive psychotherapy, Insight oriented, CBT and Motivational Interviewing
during session. The goals of therapy is to minimize symptoms, strengthen the
therapeutic alliance, improved coping skills, change of perception and attitude
and continue with medication management. I discussed various therapeutic
interventions. Patient engaged and participated in session. Patient provided with
emphatic listening and validation of emotions

-Discussed the importance of compliance with medication and follow up
appointment.

• Encouraged to maintain sobriety and medication adherence to optimize
  therapeutic stability.

Medication:

Depakote 750mg 3 tablets BID
Increased Zyprexa to 5mg PO BID.
Increased Vistaril 25mg PO TID prn anxiety
Patient has the ability/capacity appears to respond to psychiatric medications
and appears to understand the need for medications, gave consent and is
willing to maintain adherent. Reviewed potential risks & benefits, Black Box
warnings, and alternatives including declining treatment.
Patient agreed verbally to Medication monitoring Service Plan & Treatment
consent.
Patient educated of the process of psychiatric evaluation, prescription and
monitoring of psychiatric medications including the requirement of labs and drug
testing. Prescription practices of controlled substances reviewed including
termination of care in the event of diversion, abuse, doctor-shopping and use of
unprescribed medications. Also they are informed to call the clinic 72 hrs
advance for refill of their prescription.
